Telehealth consent.

How telehealth works here
Helix Restore connects you with licensed clinicians who evaluate your health information remotely — through the intake you complete online and, when needed, a video consultation. If a clinician determines a therapy is appropriate for you, they issue a prescription, and a licensed pharmacy in our network dispenses it and ships it directly to you.
What you should know before consenting
- Telehealth has limitations: a remote evaluation cannot cover everything an in-person examination can, and your clinician may require labs, an in-person visit, or additional information before authorizing any therapy.
- Submitting an intake does not guarantee a prescription. The decision to prescribe belongs solely to the evaluating clinician.
- All medications require a valid prescription and are dispensed by licensed pharmacies in accordance with the laws of your state.
- Medications can have side effects, interactions and risks. Your clinician will review these with you, and you should read all information provided with your medication.
Emergencies
Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
Your rights
- You may withdraw your consent to telehealth care at any time by contacting us; doing so may end your ability to receive care through the platform.
- You may request your health records and ask how your information is used — see our Privacy Policy.
- You may request an in-person evaluation at any time instead of continued telehealth care.
Your consent
When you check the consent box on your intake, you confirm that you have read this page, understand how telehealth care works through Helix Restore, and agree to receive care this way.
